TX Hospitals Hyderabad successfully managed one of India’s rarest twin pregnancies

Doctors at TX Hospitals, Banjara Hills, recently pulled off one of the most complex and life-saving surgeries in maternal healthcare, saving a young mother and her twin babies from a rare and dangerous pregnancy complication.

Mrs. Ayesha Siddiqua, 29, was just 28 weeks pregnant when she began experiencing heavy bleeding. What doctors found was alarming: a condition called placenta percreta, where the placenta had not only grown deep into the uterus but had invaded the urinary bladder and cervix — a globally rare scenario, especially in a twin pregnancy.

What Made This Case So Rare?

This was not just a high-risk pregnancy — it was a globally rare combination of complications:

  • Placenta Percreta: Where the placenta grows beyond the uterus, invading surrounding organs
  • Bladder Invasion: The placenta had penetrated the urinary bladder and cervix
  • Twin Pregnancy: She was carrying DCDA twins (two separate placentas and sacs)
  • Previous Cesareans: A history of two cesarean deliveries and a uterine surgery added to the complexity

This combination posed extreme risks of massive internal bleeding, organ damage, and maternal or fetal death.

The patient was first stabilized and then moved to a cath lab, where Dr. Avinash Dal, Senior Consultant – Vascular Surgeon, inserted balloon catheters in both common iliac arteries. This crucial step minimized blood loss during the upcoming surgery.

  • Dr. Sruthi Palakollu and Dr. Sudha S, Consultants – Gynaecology & Obstetrics, prepared for one of the most complex cesarean deliveries.
  • With the help of real-time ultrasound guidance, a precise incision was made — avoiding damage to the placenta.
  • Both babies — a 1 kg male and 760 g female — were safely delivered within one minute of each other.
  • Arterial balloons were inflated immediately post-delivery, controlling hemorrhage effectively.

Post-Delivery Surgical Care

The team continued with:

  • Cesarean Hysterectomy to remove the uterus.
  • Bladder reconstruction (about 20% tissue repair) led by Dr. Hidayatullah (Sr. Urologist) and supported by Dr. Rajesh.
  • Emergency GI surgeons, including Prof. Dr. DVL Narayana Rao and Dr. Anil Jampani, were on standby in case bowel involvement was discovered.
